• Male or female
• 18+ years of age
• Able to walk or wheelchair reliant
• Individuals with a cognitive disability (an IQ of 70 or less)  … OR…

Individuals (perhaps with higher IQ’s) with cerebral palsy, epilepsy, or autism
who additionally have substantial functional limitations in at least three of
following life areas:

1. Self-care
2. Receptive and expressive language
3. Learning
4. Mobility
5. Self-direction
6. Capacity for independent living
7. Economic self sufficiency

• Individuals with medical needs:
  
  
  • Injectible medications (insulin, etc.)
  • Taking multiple medications 
  • Oxygen (in-house and portable)
  • C-Paps & Bi-Paps
  • Nebulizer treatments
  • Feeding tubes
  • Colostomies & Urostomies
  • Catheterization (supra pubic, intermittent, & indwelling)
  • Non-ambulatory (wheelchair reliant)
  • Amputees
  • Anticoagulant therapy
  • Pacemakers
  • VNS  (Vagus nerve stimulator)
  • Dementia
  • Other medical needs are evaluated on an individual basis

• Individuals who have been told by their current residential provider to consider
  other options due to a change in medical condition (often something as simple as
  the individual having to begin using a wheelchair or becoming diabetic).
  
  
• Individuals desiring an encouraging, fun, high-activity lifestyle despite their
  developmental or medical challenges!
